The Ceremony Planning Form is an interactive tool that will allow you to communicate all of your wedding ceremony preferences to your parish’s Director of Music and Liturgy. It is your responsibility to contact the Director for a meeting to review and discuss your selections.
The Ceremony Planning Form must be completed in a single session, so please be sure that you have viewed your options for music, readings, etc. on this website. After the form is submitted, you will receive an email with a copy of your responses.
It is important that you have discussed with your priest/deacon if the wedding will take place during a Catholic Mass or with only the Liturgy of the Word. Please select the appropriate Ceremony Planning Form from the following list:
